I'm pretty sure they just let it go on until the guys hit the ground, then they break it up. I asked the same question to my friends who are big into hockey the other day and that's the answer i got.

it's viewed as part of the game, either a way to break/gain momentum, a way to get noticed (if it's minor leagues) and the obvious you're upset. Generally the refs let it go until someone hits the ice, sometimes they break it up if they can.
